How do I handle my annoying boss?

I work in the medical field, CT scan & X-ray. My boss, not being formally trained - merely on-the-job trained, knows less about CT than he would like to admit. I am not sure if he knows that and that is why he is constantly riding my back or if he rides it for pure enjoyment but I feel like if things don't change, I am either going to loose my mind, do something potentially huge like go to HR, or find another job (which there aren't any).
The only way to describe it is to list all the instances, so here it goes;
1. He naturally gets paid so much more than everyone else, and takes OT all the time, and it is always avoidable, and unwarranted. But no one else is allowed to get close. He says he is busy but he isn't. He usually spends time surfing the internet, working on his own personal errands, and then uses after hours to work on "work".
2. Before I came to work there, there was no organization to their department protocols, there was no governing outline to their examination protocols, and since I had experience with excel and other programs as well as typing up protocols, I did the department protocols for him. I also aligned our examination protocols with the accreditation association that keeps high standards for good practice (something he didn't even know about). Once I started all the protocols, with the letterhead and basic outline and printed them all, he made minor adjustments to all of them and now takes credit for doing them. And even though I started from scratch, it took me only a fraction of time that it is taking him to do all his adjustments.
3. He likes to skip taking a lunch in order to get easy OT. He once told me he was going to go to lunch but was stopped by a physician to take care of something, so he didn't get lunch. The next day the same physician called me to ask me why this issue wasn't taken care of. It was obvious what happened. I did ask him about the constant skipping lunch for unimportant tasks thing and he basically said it was ok for him to do it because he is the manager.
4. It would take an x-ray tech to know the irritation behind this next thing, but basically he makes very minor adjustments to positioning. For any x-ray techs out there... he made me change my SID from 47" to 40" on a KUB. For everyone else, he micromanages things, making me change things to "his" way, even if his way is not the best way.... because he's the "manager". There is no discussing it either because I just get the "because I said so!" response.
5. He prints out so much personal things. A couple of times, wasting so many pages because he didn't print preview prior to printing, which I really don't care about except that when I printed out some stuff to better our department, he complained about it. He said I shouldn't be printing out that much stuff and if so, I should have printed double sides.
I just need to know that I am not crazy, that he is doing things that are wrong, and if there is any possible thing I can do about it.

Commentary in the order posted:

1) Begin dialog with this person regarding equitable distribution of overtime. If it goes nowhere, document your hours and the supervisors with consideration of discussing this with HR.



2)In a situation where this type of person cannot be trusted, be sure to password protect ALL documents you originate with a password. Put your name on as the author and YOU do all revisions. (if need be, on overtime :-) )



3)I would bet that there is a policy against this. Most places MANDATE that you take your unpaid lunch, if you are hourly.



4)Who cares. You get paid exactly the same. You could always ask him to substantiate the change he insisted on. He may not have a logical or medically supported argument and will soon tire of embarrassing himself each time you ask a question he has no valid answer for.



5)Not much you can or should do with this one.

Not sure what to do since I never worked before but im thinking, collect some of his "personal papers" and record some things he does. Itll catch him off guard that youre recording him. Then record again with voice memos, or just a video camera that youre here, and hes not here. Also test his knowledge too. After you are done, you can hand in your evidence to get him fired. Can someone fire him? lols.
